Forms That Must Be Completed

Connecticut College Soccer Camps must have a completed copy of each of the below forms in order for campers to attend ANY of Connecticut College Soccer Camps (CESA, NE90 or Session 1 & 2). In addition to the forms, each camper must also submit a copy of his/her physical exam, which must have been completed within the last year.

Copy of the Player's Most Recent Physical (All Camps)

The physical can be emailed to info@conncollegesoccercamps.com, uploaded during registration on SportsEngine, or brought in on the first day of camp.

Self-Administer Medication

The State of Connecticut mandates the following protocol concerning Self administer prescribed medication.

Permission To Self-Administer Prescribed Medication

CONNECTICUT COLLEGE SOCCER CAMPS will ONLY allow self-administration of medication by a minor child when the permission to self-administer prescribed Medication Form has been fully completed by a parent/legal guardian and prescriber.  Connecticut College Soccer Camps are only self-administer camps.

Please note: Self-administration means that the minor may administer the medication in a manner directed by the physician.

Please complete the Authorization for the Administration of Medication form, which can be found by clicking the link below.  This form is ONLY VALID when it is filled out completely, which includes the following items:

Authorized Prescriber’s Order

  • Name of Medication
  • Dosage and Method
  • Specific Instructions
  • Medications Administration Dates
  • YES box needs to be checked after the question, Is this Medication to be self-administrated by the child?
  • Prescriber’s Name, Address and Signature

Parent/Guardian Authorization:

  • Check the box by ‘I request that medication be self-administered by my child as described and directed above.’

In addition, the following items are required pertaining to the prescription.

  • The prescription must be in its original box/container, which you received when you picked up the prescription at the pharmacy.
  • The original prescription label must be on the medication or the box.
  • The prescription must be current and not expired.
  • If your child need an over-the-counter medication (ie. Tylenol) This medication must have your child’s name labeled on the medication and it must be in the original container from the store.

On the first day of camp the prescription will be given to the medical staff, where it will be stored in a locked box for the entire camp week. 

Your child will not be able to participate in camp activities until all of the appropriate paperwork is completed, handed in and the medication follows the requirements stated above.If your child needs to administer his/her own medication, please complete the following form:
